Do I need a permit to trim a tree at my Fort Myers house?

Not if the tree is on a single-family residential lot AND you have written documentation from an ISA-certified arborist or Florida-licensed landscape architect stating the tree poses a danger to persons or property. Florida Statute 163.045 prohibits the City from requiring a permit or fee in that case. Routine pruning on private residential property is also generally unregulated by the City.

Can the City ever require a permit to trim?

Yes. FS 163.045 does not preempt the City for trees in the public right-of-way, for non-residential or multifamily parcels, or for trees that are part of an approved landscape plan under the Land Development Code. Those still require review under Chapter 138 (Vegetation).
