OC's bridge-housing model includes Bridges at Kraemer Place (Anaheim), Yale Navigation Center (Santa Ana), Be Well OC campuses, the Courtyard, and Costa Mesa Bridge Shelter. These low-barrier sites accept individuals with pets, partners, and possessions, offering 24/7 access, meals, behavioral-health services, and intensive case management. The Office of Care Coordination uses Coordinated Entry through 211 OC to assign beds. United Way OC funds wraparound services. Average length-of-stay targets are 90 to 180 days. Project Roomkey and Homekey conversions added permanent supportive housing through state HHAP funding. Bed counts factor into Martin v. Boise compliance calculations countywide.
Bypassing Coordinated Entry intake, refusing low-barrier admissions arbitrarily, or operating shelters without state HMIS reporting can jeopardize HHAP funding and Continuum-of-Care compliance.
