Whenever a permit is required by provisions in this chapter, an application shall be filed with the director stating: A. The name and address of the applicant; B. The name and address of the person, group, organization or corporation sponsoring the activity; C. The nature of the proposed activity; D. The dates, hours, park facility and location within the park for which the permit is desired; E. An estimate of number people in attendance.
