Sec. 12-388(a) requires each food truck applicant to submit the name and address of the food preparation establishment where items are prepared, that establishment's most recent health inspection report and Will and/or DuPage County Health Department permits, a description of the food and beverages offered, a letter of agreement from the preparation establishment, a proposed list of operating locations and times, a description of the truck itself, copies of any state health or transportation licenses, written property-owner consent for each site, and a signed hold-harmless and indemnification statement.

Sec. 12-395 exempts food delivery vehicles and trucks operating at temporary fundraisers or village-approved events. Operating without a valid village, county health, or state license or permit violates Sec. 12-394 and can lead to license revocation proceedings.
