Sec. 18-58(c) places tattoo shops among the businesses that need an extra sign-off before the county will issue a business license under Sec. 18-41: approval of the county board of health and a copy of the state medical license held by the physician in charge of tattooing. That means the application has to include both the county board of health's approval and documentation of the state medical license held by whichever physician is responsible for the tattooing performed at the shop. Sec. 18-59 reinforces this: any business that needs a health permit or other regulatory approval must show the county it has already secured that approval before the business tax director will issue the registration.

A tattoo shop's certificate authorizes operation at one address only; Sec. 18-54 requires the business tax director be notified in writing, no later than the date of any move, and a new certificate issued once the new location clears zoning. Like other certificates under this article, a tattoo shop's registration is not transferable to a new owner (Sec. 18-55) and automatically expires 30 days after the anniversary of initial registration, requiring annual renewal (Sec. 18-56).
