Section 18-428(c) requires three things on every unit's inspection report. First, the names of designated employees or other authorized persons assigned to respond to emergency conditions (18-428(c)(1)). Second, a telephone number where those designated people can be reached during any 24-hour period, covering emergency conditions the code specifically lists: fire, natural disaster, flood, collapse hazard, burst pipes, or violent crime (18-428(c)(2)). Third, the exact notice a tenant needs to escalate an unresolved problem to the city: "TO REPORT UNRESOLVED VIOLATIONS OF THE CITY'S MULTI-FAMILY HOUSING CODE FOR THESE PREMISES, CONTACT THE CITY OF FRISCO CODE ENFORCEMENT DIVISION" (18-428(c)(3)).

The landlord must keep these reports, notice included, on the premises of the complex, or at the landlord's central business office if there is no onsite office, and make them available to the city administrator for inspection at all reasonable times (18-428(d)). This notice sits inside the same annual walkthrough inspection report required by Section 18-428(a), so a tenant encounters the reporting instructions as part of that documented process rather than through a separate complaint form.
