Section 46-153 requires every food establishment inspection to be recorded on a town inspection report form that lists a weighted point value for each requirement, with remarks referencing the specific item violated and the correction needed. The establishment's "rating score" is calculated as 100 minus the total of the weighted point values for all violations found, and a copy of the completed report goes to the person in charge at the end of the inspection; the form is a public document available on request. Section 46-154 ties correction deadlines to both the severity of each violation and the overall score: an imminent health hazard, such as a lack of refrigeration or a sewage backup, forces the establishment to stop food operations immediately until the town authorizes a restart.

Four- or five-point violations must be corrected within ten days, one- or two-point items by the next routine inspection, and if the overall rating score falls below 60, the establishment must start corrective action on every identified violation within 48 hours, with the town free to schedule reinspections at its discretion. Temporary food establishments get only 24 hours to correct any violation before they must shut down. An operator who disputes either the findings or the deadline can request a hearing within ten days of any cessation of operations, and the town must hold that hearing within 20 days of the request.
