' The application under § 405-3 demands the applicant's name, address and Social Security number, the employer or organization represented, the merchandise or service offered, three years of residence history, any vehicle used and its registration, whether the applicant has ever been convicted of a crime, the names of other Pennsylvania municipalities worked in over the past two years, and the specific dates and locations of intended activity; it cannot be filed more than three months before the activity begins. Once complete, § 405-5 requires the Director of the Department of Licenses and Inspections (or a designee) to issue the license within 10 days unless the information is false, in which case the application is denied and the applicant faces the chapter's penalties.

Every licensee must carry the license card at all times and present it on request, may not let anyone else use it, and must immediately report a lost license under § 405-7.
