17(6)(b) sets the paperwork the City Treasurer must collect before registering a dog that has been declared or found to be a potentially dangerous dog under the section's conduct-based definitions. The Treasurer may not complete that registration unless the owner provides a current Rock County license for the dog, a color photograph of the dog being registered, proof that the dog has been spayed or neutered, and proof that the dog has been implanted with a potentially dangerous animal identification number. This spay-or-neuter requirement is triggered entirely by the potentially dangerous designation; nothing elsewhere in Chapter 15 imposes a general spay-or-neuter mandate on ordinary pet dogs or cats kept in Beloit.

The registration itself must happen within 30 days of the section's effective date and every year afterward by April 1, and the Treasurer issues a receipt as proof once all four documents are on file. Because registration and the spay-or-neuter proof it requires are gateway conditions, an owner who cannot produce the proof simply cannot lawfully register the dog, which then leaves the dog unregistrable and its owner exposed under the separate unregistered-dog penalty in subsection (7).
