Every applicant, partner, corporate officer, or director, and any stockholder holding more than 10% of a corporate applicant, must supply written proof of being at least 18 years old, three years of residential addresses, three years of employment history, prior massage business experience, any prior license denial, revocation, or suspension anywhere, and any criminal or ordinance conviction or plea within the past five years. Under subsection (D), the applicant files in duplicate under oath and pays a nonrefundable $100 filing fee; the City Controller refers the application within 15 days to the Board of Health, Fire Department, Building Department, and the administrative official, who must inspect within 45 days and decide within an added 30 days if held for investigation.

11(A) directs denial if the proposed operation would not comply with the city's building, health, planning, housing, fire, or zoning codes, if a manager or operator has been convicted of a felony or a sexual-misconduct offense involving children, or if operation would violate the chapter. Licenses run on a fixed cycle: the initial term ends the following August 31, and renewals run September 1 to August 31 each year. 12 allows revocation or suspension after at least 15 days' written notice and a public hearing before the administrative official for good cause, including a chapter or state criminal violation the licensee knew or should have known about, or a false statement on the application.
