01. 040 application must be filed with the Provo City business license office before operations begin and must list the owner's principal place of business, every vehicle to be used (with license plate, VIN, insurance and safety inspection), and any changes must be reported within ten business days. Applicants who are corporations or LLCs must submit certified formation documents, and every applicant needs a valid Utah County Health Department permit and a Utah sales tax certificate naming Provo as the city of operation. 040(6) sets minimum liability insurance at $200,000 per person, $500,000 per occurrence and $500,000 for property damage, with the City named as an additional insured and 30 days' notice required before cancellation.

040(8) requires an annual fire inspection by the Provo Fire Marshal's Office, a tagged 5-pound ABC extinguisher, a UL-listed Type I hood with suppression system for grease-producing units, GFI outlets within six feet of water, and bans storing propane inside the cooking or passenger area. 070(2) limits right-of-way vending to no more than three days in any seven-day period, with vehicles removed from the right-of-way at the close of each business day. 030(6). 040(4) that turns up a DUI conviction within ten years or a violent, weapons or drug-distribution conviction.
