(a)No person shall locate, establish, set up or operate a carnival or an itinerant meeting unless he has an unrevoked and unexpired permit from the health department authorizing him to do so. The application for such a permit shall be submitted in writing a minimum of 30 days prior to the event and shall contain (in so far as applicable) the following information:(1)Proposed site;(2)Estimated area to be occupied;(3)Number of persons traveling with or employed by the carnival or itinerant meeting;(4)Number of animals carried;(5)Number of days proposed site to be occupied;(6)Number of itinerant restaurants to be established;(7)Number of food handling employees;(8)General layout of food vendors;(9)Location of water and sewer utilities;(10)Any other information the health department may require.(b)This permit may be temporarily suspended by the health department if the holder violates any of the terms of this article, and it may be revoked upon serious or repeated violations after a hearing by the board, or the appropriate committee thereof.
