D sets the document package every cannabis-establishment applicant must submit to the Cannabis Control Board, and two items apply specifically to Class 5 cannabis retailers, the storefront dispensaries. 13 requires a list of any other Class 5 Cannabis Retailers operating within 600 feet of the main entry door of the applicant's proposed location, obtainable from the Division of City Planning, which must supply it within 30 days; if Planning misses that deadline, the Board can still hear the application on proof the request was made.

14 requires a list of all schools within 1,000 feet of the proposed Class 5 retail location, measured against the Drug-Free School Zone map maintained under § 151-1 of the municipal code. 12 layers on a third distance-based disclosure specific to Class 5 retail: a list of child care providers and substance-abuse treatment facilities within 200 feet, obtained from the Department of Health and Human Services within 30 days of a certified mailed request. 16, the Board may require a Class 5 applicant to directly address proximity concerns raised by any of these lists, to a substance-abuse facility, day care, school, or another Class 5 retailer, and factor the applicant's response into its Local Licensing Requirements evaluation before recommending a resolution of local support to the Municipal Council.
