Section 615.015 requires an annual body art establishment license before anyone operates a body art establishment, and applications go to the Business Licensing Department with the applicant's name and home address, the establishment's address, a copy of any State Division permit, the annual fee from the Section 605.013 schedule, and any other information the Health Supervisor requires; the license cannot be transferred between locations, people or entities. A separate body artist license, also annual, is required to practice body art procedures, with the same application elements plus proof of meeting State licensing requirements under Title 20, Division 2267, Chapter 2 of the Code of State Regulations.

Any business using an ear-piercing gun or similar device, including a jewelry store offering ear lobe piercing as a convenience, must hold a body art establishment license, though jewelry-store employees performing only ear lobe piercings are exempt from the body artist apprenticeship rules. Section 615.020 sets the annual license fee for both establishment operators and body artists per the Section 605.013 schedule. Section 615.025 ties City renewal to the State: once the Missouri Division of Professional Registration renews a practitioner's or establishment's license under Sections 324.520 to 324.526, RSMo., the City renews its own license on receipt of a completed renewal application and fee. Chapter 615 also carries detailed sterilization, sharps-disposal, biomedical-waste and jewelry-standard rules enforced by the City's Health Department.
