(1) Application for use of a park, recreation area, or other public property for a rally, meeting, or other expressive activity shall be made to the parks and open space department at least 21 days prior to the event, with a copy sent to the police department, and shall contain the following information:
a. A description of the event.
b. The day, hour, and length of the event.
c. Location of the event.
d. A reasonable and good-faith approximation of the number of persons expected to attend the event.
e. The name, address, and telephone number of the person conducting the event.
