Scottsdale Code § 16-206 makes a license mandatory for any massage facility, with narrow carve-outs for hospitals, licensed physicians' and chiropractors' offices, athletic trainers, licensed barbers or cosmetologists doing head/face/hand/foot work only, massage students in clinical practicum, and specialized weight-reduction techniques. The director will not license a mobile massage unit to operate on a city street, though the city may authorize one for a special event under § 16-206(d). citizen or authorized worker, and free of any offense under Article I of Chapter 16 within the past five years, any voluntary license surrender under investigation, sex-offender registration, or a prostitution or child sexual-misconduct conviction within the past five or ten years.

Section 16-207 sets the initial and renewal fee at $300 annually, a $100 initial application fee, a $10 city fingerprinting fee, and a $50 fee to change locations; fees are nonrefundable except as state law requires. Section 16-205 requires fingerprints from every applicant, controlling person and agent, valid for 90 days across related applications. Facility standards in § 16-210 require 400-lumen minimum lighting, sterilized instruments, separated clean and used linens, and a dressing room with a toilet and wash basin within 150 feet if not on-site.
