The Arizona State Board of Massage Therapy licenses individual therapists statewide under ARS Title 32 Chapter 42. Maricopa County requires a transaction privilege tax license but does not separately license massage establishments in unincorporated areas beyond zoning.
Arizona regulates massage therapists through the State Board of Massage Therapy, which sets education hours, exam, and renewal rules under ARS 32-4201 et seq. Therapists must be 18, complete 700 hours of training, and pass MBLEx. Maricopa County requires zoning clearance for spas in unincorporated areas and a state TPT license for sales tax. Phoenix, Mesa, Tempe, Scottsdale, Glendale, and Chandler add city massage-establishment licenses with background checks and operating-hour rules to combat illicit operations. Establishment owners must verify each therapist holds a current state license. Random inspections are common in cities with histories of human-trafficking concerns.
Practicing without a state license is a Class 1 misdemeanor under ARS 32-4253, fines up to $2,500 and six months jail. The Board may impose civil penalties up to $1,000 per offense. Cities can suspend establishment licenses.
