Arizona regulates massage therapists through the State Board of Massage Therapy, which sets education hours, exam, and renewal rules under ARS 32-4201 et seq. Therapists must be 18, complete 700 hours of training, and pass MBLEx. Maricopa County requires zoning clearance for spas in unincorporated areas and a state TPT license for sales tax. Phoenix, Mesa, Tempe, Scottsdale, Glendale, and Chandler add city massage-establishment licenses with background checks and operating-hour rules to combat illicit operations. Establishment owners must verify each therapist holds a current state license. Random inspections are common in cities with histories of human-trafficking concerns.
Practicing without a state license is a Class 1 misdemeanor under ARS 32-4253, fines up to $2,500 and six months jail. The Board may impose civil penalties up to $1,000 per offense. Cities can suspend establishment licenses.
