Sec. 14-6 requires an annual application to the township clerk covering fourteen items: the applicant's names and addresses for the past seven years, a state photo ID, the parcel's zoning classification, copies of each massage therapist's state license, a local agent for service of process, prior businesses owned, past nuisance or license-revocation findings, any specified criminal convictions, an ICHAT criminal background report paid for by the applicant, and proof of $250,000.00 in general liability insurance. Sec. 14-7 routes the completed file to the police, building, planning, treasurer, fire, water utilities and code departments, each with thirty days to recommend action, while the chief of police separately has thirty days to investigate criminal and regulatory history.

Sec. 14-8 lists fourteen grounds for denial, from missing information to unpaid taxes, uncorrected code violations, or a prior nuisance finding, and Sec. 14-10 requires the clerk to approve or deny within ninety days. Licenses expire every March 31 under Sec. 14-12. Sec. 14-30 sets facility standards, including a posted human trafficking notice under MCL 752.1035 and at least a sixty-watt light in every treatment room, while Sec. 14-31 bars operation between 10:00 p.m. and 7:00 a.m., bars anyone under eighteen without signed parental consent, and prohibits alcohol and controlled substances on site. Sec. 14-17 through 14-19 govern revocation and a twenty-day appeal to the township board.
